Food Permission Form

Pursuant to 65C-22.005(1)(c)2., F.A.C. licensed child care facilities must obtain written permission from parents/guardians regarding a child’s participation in food related activities. Please download, print, fill out and bring us this form if you would like your child to take part in food activities such as classroom cooking projects, gardening, school wide celebrations, and birthdays.
